Implantology

Implantology is the fastest growing field of dentistry. In our practice we work with implants for many years, allowing patients to replace missing teeth without interfering with healthy teeth.

Implants also provide stable support for partial and complete dentures. In implantology, cooperation between surgeons and prosthodontists is crucial, as together they plan and execute the treatment process to achieve optimum aesthetic and functional results.

In order to reduce the duration of the procedure and carry it out more precisely, we use surgical templates to precisely insert the implant into the planned place. Implant navigation turns a good implantologist into an excellent one.

Implant

It is a new root that is inserted in place of a previously extracted tooth. The implant is made of titanium, which is treated by the body as if it were its own bone. After a few months, it grows together in the same way that broken bones grow together.

Before the procedure, the doctor assesses the health of the mouth and recommends an X-ray. The basic examination by which the doctor assesses the quantity and quality of the bone is a pantomographic picture. In more complex situations, a CT scan of the bone fragment of interest is performed.

The procedure lasts approximately 60 minutes and is performed under local anaesthesia, which is sufficient to make it completely painless. The implant procedure is a more delicate procedure than tooth extraction. After a few months in the laboratory, a technician rebuilds the crown of the tooth, which is identical in appearance to the patient’s other teeth.

Not enough bone material

Too little bone surface is a fundamental problem in implantology. Currently, thanks to the dynamic development of knowledge on the subject, bone augmentation procedures called guided bone and tissue regeneration (GBR, GBT, osteodistraction) are being performed. These procedures involve the introduction of a special preparation – granules – which provide a scaffold for the newly forming bone, which grows in between the material and stabilises the entire structure. The granules with the newly forming bone are separated from the gingiva by a special membrane – a collagen membrane – to ensure proper bone healing. An amount of bone the size of a 0.2 PLN coin is required for a fully predictable treatment result.

How long does the implant last in the mouth?

A properly placed implant, fuses with the bone after a few months. The lost tooth is then rebuilt in the implant. The prosthetic structure restored in this way fulfils its function as long as the bone supports the implant. No maximum implant lifetime is specified. Proper daily oral hygiene, as well as twice-yearly check-ups at the dentist’s office, which provide an opportunity for early detection of abnormalities, are essential for a long-term positive treatment effect. The first implants were placed in dental surgeries in the 1970s and are still in use today.

What is the failure rate?

Implant treatment is not an experimental treatment. It is a very well researched and documented method of replacing missing teeth. Continually improving implant technology and current medical developments have resulted in a treatment success rate of at least 96%.

What happens when the implant fails to fuse with the bone?

Implant failure requires a change of concept to surgical or prosthetic treatment. Usually, the implant is removed and a second implant is inserted at the same appointment, or treatment is carried out to increase the volume and quality of the bone. The removed implant is sent to the manufacturer, who carries out an analysis of the cause of failure. Implant failure is not dangerous to health and only requires local measures to change the placement of the new implant.

What are the contraindications to an implant?

The current assessment is that there are no situations in which it is absolutely impossible to place a dental implant in a patient. There are diseases (diabetes, osteoporosis, cardiovascular diseases, joint diseases, connective tissue diseases) that require postponing the treatment until the patient’s condition improves. In such a situation, the patient’s condition is always consulted with a specialist in the relevant field and treatment is carried out together with him or her.

Does smoking have an effect on an implant?

In smokers, the risk of implant rejection increases by 10 per cent. Research on this problem shows that one of the main problems is the negative effect of nicotine on the connective tissue. Smoking has also been shown to affect the quality of the bone and thus shorten the life of the implant.

How many implants should be placed in the case of complete absence of teeth?

In the case of edentulous patients, implants are used as stabilising elements for the patient’s removable denture. It is then sufficient to place 2 to 4 implants in one jaw and the prosthesis will be completely immobile. If a prosthetic restoration is designed that is permanently connected to the implants, 8 implants are required in the jaw and 6 implants in the mandible. With implants, chewing function is completely restored. The bone is constantly stimulated and does not atrophy, but on the contrary regenerates. Food can be properly broken down, preventing serious systemic complications associated with poor nutrition.
